At the risk of waxing all Itrawebz0r MD, it sounds a bit like fibromyalgia. It comes and goes, causes a lot of muscle pain with fever, and is a very bitch to diagnose.
Of course it could be a million things, and I'm only clicking on fybromyalgia because I had it. But for what it's worth...
